In the original Garfield strip, the spider was talking to Garfield about Uncle Leon, who was healthy his whole life. Then one day, there was a BOOM, that Leon went just like that. Garfield asks the spider if it was a heart attack, but the spider replies that it was a Hong Kong phone book.
Here, the title is a reference to Death Note, the question mark from Garfield's thought bubble on "heart attack" has been replaced by an exclamation point. Garfield was shocked about the spider's news here.
The spider exclaims it was Kira.

This strip's "heart attack" line really made me think of what would happen if someone were to only write down their intended victim's name in a Death Note.
